Instructions for Completing Consumer Loan Application


IMPORTANT INFORMATION ABOUT PROCEDURES
FOR OPENING A NEW ACCOUNT

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask you for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

Instructions for completing Consumer Loan Application

  1. Indicate below the amount of loan requested, the term requested, and purpose of the loan. (Please mail or fax two most recent pay stubs and/or two years tax returns if self-employed.)
  2. Complete all information requested in the application.
  3. If the loan will have a co-applicant, both must complete a Loan Application.
  4. After completing, you may print the Loan Application form and send it via "snail mail" to us at: First Hope Bank, 220 Woodport Road, Sparta, NJ 07871
  5. You can also FAX it to us at (973) 729-6201.
  6. After the application is processed, you will be contacted regarding the status of your loan application (approved/denied/additional information required).
  7. If approved, we will contact you to arrange to have the necessary loan documents signed.
